Abstract
Colorectal Cancer (CRC) is one of the most commonly diagnosed cancers in the United States. More than 140,000 patients in the United States will be diagnosed with CRC in 2020. Despite clear benefits to timely CRC screening only 63% of eligible patients in the United States are screened appropriately. The Target Health System has identified that under-screening for CRC posess a significant patient care outcome and quality reimbursement concern. This evidence-based practice (EBP) quality improvement (QI) project was designed to improve the CRC screening rates in primary care. The EBP QI project used the existing care coordinator program to expand outreach, identify patient preference, address barriers to screening, and increase access to testing. The PICOT question: Among patients 50-75 years of age eligible for CRC screening (P), how does comprehensive care coordinator telephonic outreach (I), compared to current practice of mailing a standard letter about CRC screening (C), affect 1) CRC screening rates, 2) Utilization of Target Health System colonoscopy services 3) Reimbursement of colonoscopy services, and 4) Impact of screening (O), over a 90-day period (T)? One-hundred patients, 50-75 years of age received comprehensive care coordinator telephonic outreach. Project outcomes demonstrated a 5-fold increase in CRC screening rates, a significant retention of services within the Target Health System, and an improvement in colonoscopy screening resulting in a projected significant improvement in reimbursement. The impact of screening was significant with a 4-fold increase in colonoscopy screening and more than 70% of individuals (n=5) received surgical excision of polyp(s) during colonoscopy services.
