Abstract
Background of Project:
- Stroke is the leading cause of disability in the United States.
- Caregivers are a vital component to successful discharge.
- Inpatient rehabilitation facilities (IRF) are designed to prepare individuals and their caregivers for the transition to home.
- IRF’s measure their success using post discharge surveys.
- The IRF involved in this project measured discharge preparedness against a benchmark score.
- A low score of 3.03/5.0 with a benchmark of 3.4/5.0 resulted in the creation of a key performance initiative regarding discharge preparedness.
The purpose of this project is to utilize an evidence-based tool in the assessment of stroke caregiver preparedness for discharge. Understanding the caregiver needs will improve the nursing and interdisciplinary team education and training of stroke caregivers, which may reduce caregiver burden and improve transition from rehabilitation to home.
