Leading Through the TEMPEST: A Place-Based Framework for Climate, Health, Equity and Healthcare Systems [Title Slide]

Abstract

Purpose: The TEMPEST Framework- Transdisciplinary Equity & Medicine for Preparedness, Environment, Systems and Transformation was developed in response to gaps identified in 10 international frameworks assessing health consequences of severe weather events. Prior models demonstrate strong integration of direct, indirect, mental, and equity-related health impacts (Boyal et al., 2018; Birnbaum et al., 2015) but remain limited by contextual specificity, data gaps, and lack of empirical validation (Hunter et al., 2016; Smith et al., 2022; Gonah et al., 2025). TEMPEST is an innovative and evidence-informed model advancing the World Health Organization’s (WHO) Operational Framework for Climate-Resilient Health Systems (CRHS) (World Health Organization, 2021). The framework unites these strengths into a generalizable, equity-centered model to guide climate-health research, policy, and nursing leadership aligned with Sustainable Development Goals (SDGs) 3 (Health) and 13 (Climate Action)(United Nations, 2023).

Methods: Development of the framework involved synthesis of literature across climate resilience, disaster medicine, and health-systems leadership. Concept mapping and iterative validation guided organization into five domains: (1) climate drivers and hazards; (2) exposure and susceptibility incorporating equity filters; (3) WHO CRHS capacities- governance, financing, workforce, products and supply chains, information, and service delivery/infrastructure (WHO, 2021) ; (4) multi-domain outcomes-direct, indirect, mental, and cascading; and (5) SDG-mapped adaptation actions within a continuous learning and evaluation loop (UN, 2023).

Results: TEMPEST addresses the limitation noted in existing frameworks by incorporating temporal phases (Birnbaum et al., 2015), structural inequality pathways (Smith et al., 2022), and population-level adaptability across diverse contexts (Lee et al., 2020; Butsch et al., 2023; Mavrodaris, 2023). The framework provides measurable linkages between hazards, resilience capacities, and outcomes, enabling systematic evaluation of preparedness, continuity of care, and equity.

Conclusion: The TEMPEST Framework offers a novel, globally relevant foundation for research, education, and practice that transforms climatic disruptions into adaptive strength. It transforms the fragmented landscape of prior frameworks into a cohesive structure for global adaptation, policy, and interdisciplinary research.
